The Challenges of Medication Reconciliation

Organizations are challenged to develop a standardized framework to reduce potential harm related to medication reconciliation.  Various factors impact accurate completion of the patient's home medication list.  The electronic health record can be a tool to enhance accuracy, however understanding, education, and practice must be consistent.  The Joint Commission requires a good faith effort from the practitioner.  Focus areas should be on admission, transitions of care, and discharge.
